Read the General Information Booklet

The General Information Booklet is required reading for anybody going to Haiti. It explains all the rules, requirements and guidelines you will need to follow on your trip and what you need to do to prepare.

When you have decided on a trip, note the departure and return dates (you will need to enter them into the on-line application). Please be sure to submit your application at least 3 months before the scheduled departure date.

Complete the On-line Mission Application

After submitting your application someone from Northwest Haiti Christian Mission, Canada will contact you to finalize your trip.

Submit additional documentation

Please return the following documentation to Northwest Haiti Christian Mission, Canada by mail to our mailing address below.

• Photo copy of the picture and information pages of your passport
• If this will be your first trip with Northwest Haiti Christian Mission, Canada we will require a signed letter of recommendation from your pastor or a leader in your church on church letter head.
• Anyone forming part of the medical staff (doctors, nurses, dental hygienistsetc.) will need to provide a copy of each of the following: medical license, drivers license, birth certificate, any specialized license within your medical field
• If you are under the age of 18 we require special parental consent. Please contact us for details.
